If you would like to continue with the pre-screening process, you will need to agree to the following:

• Provide your personal and contact information, inclusive of personal health information, to Natera for purposes of allowing Natera to contact you to confirm eligibility and provide more information on the study.
• Allow Natera to confirm your identity.
• Allow Natera to store your personal and contact information provided during the pre screening for a minimum of 6 years. After that time your information will be destroyed.
• Your participation in this pre-screen process is voluntary. Natera will not condition any treatment, payment for services or eligibility for any services, current or future, on whether you participate in the pre-screen process or the study.
• You have a right to obtain a copy of your authorization.
• You can change your mind and stop at any time during this pre-screening process. If you change your mind contact 1-650-769-7787 or inform Natera’s Site Team at the time they contact you.
• By clicking the agreement button below, I understand and agree to continue in the pre-screening process and authorize the use of my personal information as stated above.
